Comments:
An unannounced renewal inspection was conducted today from 10:45am-1:15pm. There were 8 children (26 points) supervised by 3 direct care staff. The home, 3 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed doing an art project outdoors, painting with bubble wrap, sticks, wood chips, and their fingers; eating lunch, and tranisitioning to naptime. Diapering procedures were also observed. Lunch served was chicken soup with potatoes, carrots, onions, tortilla chips, and rice; apples, and milk. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, 1 of 5 children's records did not contain documentation of a completed physical form signed by a physician. Evidence: Child #3's (start date: 12/5/2018) record did not contain documentation of a completed physical form signed by a physician.

Plan of Correction: I have already talked to mom and she will request a copy from the pediatrician.

Standard #: 22VAC40-111-180-A
Description: Based on record review, 1 of 3 caregiver records did not contain documentation of a current report of tuberculosis screening form. Evidence: Caregiver B's (date of hire: 4/2017) most recent documented TB screening was dated 4/11/2017 (expired: 4/11/2019).

Plan of Correction: The caregiver will go get the TB test done by next Monday.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the family day home provider did not ensure that all areas and furnishings of the family day home were maintained in a clean, safe, and operable condition. Evidence: 1. The outdoor swing set had a blue plastic swing seat that was crack and along the middle edges of the seat, causing the seat to bend. 2. The outdoor back wooden deck area had a wooden board that was cracked and deteriorating. 3. A tree in the outdoor play area had plastic tubing around it that was cracked and no longer secured in the ground.

Plan of Correction: We will replace the wood with the crack. In the meantime, we will put tape over the cracks in the wood and on the plastic tubing. We will also replace the swing seat.
