An unannounced Focused Monitoring Inspection was conducted on May 2, 2019. At the time of entrance there was 1 provider and 9 children with a total of 16 points. The following areas were reviewed as a part of the inspection, Administration, Caregiver Training, Physical Equipment, Care of Children and Emergencies. Children were observed playing outside and watching television. The provider is no longer allowing children to use the outdoor wooden playground equipment. The inspection began at approximately 11:20 am and ended at 12:30 pm. The results of the inspection were reviewed with the provider during the exit interview. Areas of non-compliance are identified on the violation notice. The inspection results will be reviewed for license issuance. Description: Based on observation on observation and provider interviews, the provider did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection on May 2, 2019, the provider indicated it was taken down about two weeks ago because they were working on fixing the violations.

Plan of Correction: The provider will put the violation notice back up.
