Comments:

The following inspection took place between the hours of 8:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Chantilly, Virginia. At the start of this unannounced monitoring inspection there were a total of 8 children (25 points) with 2 caregivers. A total of 11 children?s files, 2 staff files, and 5 household member?s files were reviewed. The children were engaged in the following: Games, center play within the childcare area, diapering, bathroom breaks, hand washing, nap time, lunch, music, song, story time, and other organized activities. Standard #:	22VAC40-111-780-B
Description:	Based o)n review, 2 of 11 children's files reviewed indicated that the review with the parent of their emergency contact information (as required in 22 VAC 40-111-60 B 2 to ensure the information was correct, and that the parent's signed acknowledgment of the review was obtained) was not conducted. Evidence - 1. On the date of inspection (04/30/2019), documentation that a review was conducted within the past year was not available for: Child #3, and Child #10. 2. The last documented review for Child #3 was dated - 07/11/2017. The last documented review for Child #5 was dated - 04/05/2018.
Plan of Correction:	All children's files shall be reviewed on an annual basis and updated as needed. Such review shall be indicated and/or acknowledged by the date and a signature from the child's parent.
Standard #:	22VAC40-111-100-A
Description:	Based on review, 2 of 11 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (04/30/2018), documentation of a physical examination was not on file for: Child #9, and Child #10. 2. The start date for Child #9 was listed as - 10/22/2018. The start date for Child #10 was listed as - 01/02/2019.
Plan of Correction:	Documentation of a physical examination shall be obtained and placed on file for both Child #9, and Child #10.
Standard #:	22VAC40-111-90-A
Description:	Based on review, 2 of 11 children's files reviewed indicated that the Provider did not obtain documentation that each child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before a child may attend the Family Day Home (FDH). Evidence - 1. On the date of inspection (04/30/2019), immunization documentation was not available for review for the following: Child #6, and Child #7. 2. The start date for Child #6 was listed as - 04/18/2019. The start date for Child #7 was listed as - 09/05/2018.
Plan of Correction:	An immunization page shall be obtained and placed in each child's file.
Standard #:	22VAC40-111-90-C
Description:	Based on review, 1 of 11 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (04/30/2019), the immunization documentation on file for Child #5 was older than six months. 2. This immunization documentation was dated - 10/12/2018.
Plan of Correction:	An updated immunization page for Child #5 shall be obtained and placed on file.
Standard #:	22VAC40-111-590-A
Description:	Based on observation and interview, an infant was not placed on their back when sleeping or napping. Evidence - 1. On the date of inspection (04/30/2019), at approximately 10:40 am, Child #10 (6 months) was observed sleeping on their stomach in a crib. 2. The Provider stated (in an interview conducted on the date of inspection) that the Parent's of Child #10 requested that Child #10 be put to sleep in this manner. 3. There was not a written statement signed by the child's physician authorizing the placement of the infant in this manner for both sleeping and/or napping.
Plan of Correction:	A written statement from the child's physician shall be requested and placed on file.
