Comments:

A announced monitoring inspection was conducted with the provider and one assistant. Eight children were in care with a total of 21 points. The age range of the children was seven months to three years old. Children were observed during rest time and free play. All required paper work was posted and provider does not give medications. Standard #:	22VAC40-111-420-A
Description:	Based on observation the room used for rest did not have any light. Evidence: The room used for resting children was completely dark and the children could not be seen.
Plan of Correction:	I will leave the door open and leave the hall way light on .
Standard #:	22VAC40-111-510-D
Description:	Based on observation not all children were sleeping on rest mats or cots. Evidence: one child was resting on blankets on the floor.
Plan of Correction:	We will always use cots for the children to sleep on.
Standard #:	22VAC40-111-590-B
Description:	Based on observation and interview with the provider one infant was sleeping in a swinging bassinet. Evidence: One infant was sleeping in a swing bassinet and was not moved to a crib. Provider stated to LI the infant had been asleep for 30 minutes.
Plan of Correction:	We will move infants to cribs.
