63.2 Facilities and Programs
An unannounced focused monitoring inspection was conducted today from 9:50pm -11:20am due to the Provisional status of the current license. There were 12 children (31points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 1 household member?s record were inspected. The inspector reviewed the violations from the previous inspection for compliance. Children were observed working on crafts, playing freely and eating snack. An exit interview was held with the provider at the conclusion of the inspection. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-100-A Description: Based on record review, it was determined that the family day home did not obtain a physical examination for a child in care within 30 days of the child's first day of attendance. Evidence: Child#5's record does not contain documentation of a physical examination by or under the direction of a physician. The child's first day of attendance was documented as 1/24/2019. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-111-120-B Description: Based on record review, it was determined that the family day home did not obtain two or more references as to character and reputation as well as competency before employment. Evidence: Staff#2's record does not contain 1 of 2 reference checks required before their first day employment. Plan of Correction: We will obtain a second reference today.
Standard #: 22VAC40-111-90-C Description: Based on record review, the family day home did not obtain documentation of additional immunizations once every six months for a child under the age of two years. Evidence: Child#1's record contains an immunization record that was last updated on 8/12/2018. Plan of Correction: Corrected during the inspection.
