The inspector conducted an unannounced focused inspection on April 26, 2019 from 9:00 AM to 11:00 AM to examine the areas of noncompliance identified in the Intensive Plan of Action. Observations were conducted, records were reviewed, and emergency supplies were observed. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-111-900-A Description: Based on observations and interview, a food item was served to a child under four years old that was not prepared in a way to prevent choking. Evidence: The licensing inspector observed a two year old child in care who was served a turkey sausage link wrapped in cooked batter in the style of a corn-dog. The food item had been cut in half once, across the middle, but not lengthwise. The cut food item was served to the child to eat with their hands while seated. To assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery such as whole hot dogs sliced into rounds, nuts, seeds, raisins, uncut grapes, uncut raw carrots, peanuts, chunks of peanut butter, hard candy, and popcorn shall not be served to children under four years of age, unless the food is prepared before being served in a manner that will reduce the risk of choking, i.e., hot dogs cut lengthwise, grapes cut in small pieces, and carrots cooked or cut lengthwise. Plan of Correction: I have removed this item off the menu for now. In the future if I choose to serve a corndog it will be cut lengthwise. This action was corrected before licensing inspector left the home.
Standard #: 22VAC40-111-50-A Description: Based on observation and interview, the provider did not ensure the family day home shall keep a written record of children in attendance each day. Evidence: 1. The licensing inspector observed the attendance list at approximately 9:10 AM. The licensing inspector observed there were five children in care and four children listed on the attendance sheet. 2. The provider identified the child who was missing from the attendance sheet and stated the identified child arrived to the family day home at approximately 8:00 AM this day. Plan of Correction: The licensing inspector checked the USDA roster but I keep a daily log in my possession in case of an emergency. Also, I check the roster at 12 to make sure all parents signed in. This method did not work so after each child is dropped off, I am physically verifying it before the parent leaves the home.
Standard #: 22VAC40-111-60-A Description: Based on review of five children's records, the family day home did not have an up to date record for each child in care. Evidence: 2. The record for Child #2 (unknown start date) did not contain documentation of information for parents, emergency medical authorization, policy for the administration of medications, or complete emergency contact information. 3. The record for Child #3 (start date 12/3/18) did not contain documentation of information for parents, emergency medical authorization, medical insurance information, or complete emergency contact information. 4. The record for Child #4 (start date 8/6/18) did not contain documentation of information for parents, emergency medical authorization, provisions for the emergency preparedness plan, or complete emergency contact information. Plan of Correction: My initial application was one that I set up for my parents. Upon inspection I have done away with my application. I will be using a social service application from this day forward. Majority of the information was missing because the parents filled out my application. I am thoroughly going through the packets again. It will be done and completed 15 days from the date. All parents have been notified of missing information and I have given 10 days to have all packets updated with all documents.
Standard #: 22VAC40-111-240-A Description: Based on observation and interview, the provider did not ensure that areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. Evidence: 1. The licensing inspector observed a broken hot water faucet knob in the bathroom used by children. 2. The provider reported the children had access to warm running water due to the provider using a wrench to turn on the warm water as needed. The provider reported the knob had broken the previous day. Plan of Correction: Upon closing I went to Lowes and purchases a hot water faucet knob. This problem has been corrected upon the day of the inspection.
Standard #: 22VAC40-111-240-B Description: Based on observation and information from the consumer product safety commission, the provider did not ensure that recalled products shall not be used. Evidence: 1. The licensing inspector observed one Fisher-Price Rock 'N Play seat during the inspection which was available for use. The equipment had been recalled and identified as hazardous on 4/12/19. 2. The provider reported having no previous knowledge of the equipment recall and reported not being aware of the requirements for providers to prevent the use of recalled equipment. Plan of Correction: The recalled equipment was removed from the home upon licensing inspector departure. In the future I will check the news at least twice a week for recall items.
Standard #: 22VAC40-111-280 Description: Based on inspection of the home, the provider did not ensure all hazardous substances were kept in areas inaccessible to children. Evidence: The licensing inspector observed the following substances in areas accessible to children: disinfectant wipes sitting on a desk in the main child care area, aerosol deodorizing spray sitting on a shelf in the main child care area, and bottles of hand santizer labeled "keep out of the reach of children" and "flammable" sitting on a desk in the main child care area. Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children. Plan of Correction: All of products was removed immediately from the daycare area. I have changed my hours of cleaning so that no cleaning products will be mistakenly left in daycare. Also, I purchased a travel size bottle of hand sanitizer which will be stored in my pocket in the future.
Standard #: 22VAC40-111-530-G Description: Based on observation and interview, the provider failed to ensure that children under two years of age did not use pillows. Evidence: Upon arriving to the home, the inspector observed and infant sleeping in a pack-n-play with a boppy pillow (shaped as a crescent) around the child's head. The provider was asked about the sleeping environment for the infant and the age of the child. The child is nine weeks old. Children under two years of age shall not use pillows. Plan of Correction: Immediately the child was placed in the baby bed free of pillow and blankets. The date of correction was done before licensing inspector left the home. There will be no items in the area where babies are sleeping in the future.
Standard #: 22VAC40-111-550 Description: Based on observation and interview, the provider did not meet the requirements for play pen use. Evidence: The licensing inspector observed an infant sleeping in a pack-n-play playpen with a bassinet insert upon arriving at the home. 2. The provider reported the child was nine weeks old and the child's parent requested the use of the pack-n-play for sleeping. 3. The infant was moved to a crib during the inspection. Plan of Correction: The child was removed from the play pen used as a basinet while the licensing inspector was in the home. In the future no playpens will be used as basinets.
