An unannounced renewal inspection was completed on May 3, 2019 from approximately 1:00p.m. to 3:00p.m. Eleven children, ages two through four years old, were in care for a total of twenty-nine points, with two caregivers. Staff and children were observed in the indoor space engaged in activities to include nap time, waking up from nap, toileting, hand washing and preparing for snack. Interviews and interactions were conducted and no medications are given. Five children?s records, four caregiver records and two household member records were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left.

Standard #: 22VAC40-111-940-A
Description: Based on observation, the licensee failed to ensure that temperatures shall be maintained at or below 0 degrees Fahrenheit in freezer compartments.
Evidence: 1. The freezer temperature was observed at 10 degrees Fahrenheit. 2. Caregiver #1 acknowledged the temperature was not at or below 0 degrees Fahrenheit.

Plan of Correction: The provider turned the freezer temperature down on site.

Standard #: 22VAC40-111-70-B
Description: Based on review of records and interview, the licensee failed to obtain the parent's written acknowledgement of the receipt of information required to be provided in writing before the child's first day for one of five children.
Evidence: 1. Child #2's record, enrolled on January 14, 2019, lacked written acknowledgement that the parents had been provided the required information in writing. 2. Caregiver #1 indicated that the child's parents had been provided the information prior to the child's first day via email, but she had not yet obtained the signed written acknowledgement.

Plan of Correction: The provider will obtain the signed form from the parents.
