Discussed placing diaper creams in a cabinet on the wall.

Comments:
An unannounced renewal inspection was conducted today between the hours of 9:00 am through 10:30. There were a total of 10 children (30 points) with 3 caregivers. A total of 5 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, puzzles, math games, diapering and hand washing. Standard #: 22VAC40-111-100-A
Description: Based on a review of children record, there is no documentation of a physical examination within 30 days after the first day of attendance. Evidence: Child #1, date of attendance 6/11/18 does not have documentation of a physical examination.

Plan of Correction: I will contact the parent to obtain a copy of the physical examination.
