5/2/2019 Arrived 1:15pm Departed 3:45pm An unannounced renewal inspection was conducted today. Two children were present for a total of 4 points. The children were eating a snack, resting and playing with blocks. Three children's records and four adults records were reviewed. Thank you for your cooperation. Standard #: 22VAC40-111-260-E Description: Based on observation, the facility failed to ensure that the wading pool was emptied after each use. Evidence 1. The small plastic wading pool in the back yard contains four inches of water. Plan of Correction: The provider reported that the pool will be emptied after each use. It was not used for wading, it was used for a water pouring activity.
Standard #: 22VAC40-111-460-C Description: Based on a review of documentation and an interview with the provider, the facility failed to ensure that all animals are vaccinated for rabies. Evidence: 1. A stray dog has been adopted by the family and has not a rabies vaccine. Plan of Correction: The provider reported that there is an appointment this Saturday May 4 to have a physical exam and rabies vaccine.
