Comments:
An unannounced monitoring inspection was conducted today (5/6/19). There was one caregiver present with 7 children (19 points). The substitute caregiver does have completed background searches on file; however, she does not meet the requirements for a substitute, as she does not speak, read or write in English. Violations:
Standard #: 22VAC40-111-130
Description: Based on statement of the substitute caregiver, the caregiver is not qualified to be a provider. Evidence; The caregiver left alone with children doe not speak, read and write in English.

Plan of Correction: I will make sure that my assistant takes training and classes so that she can speak and write in English.

Standard #: 22VAC40-111-570-A
Description: Based on direct observation, the provider did not ensure that each caregiver did not exceed 16 points per caregiver. Evidence: On May 6. 2019 at LI arrival (1:00 PM) there was one caregiver present with 19 points. Children's ages present: 2- 2 year old children, (4 points), 2- 3 year old children (4 points); 1- 22 month old (3 points); 2- infants (8 points) totalling 19 points.

Plan of Correction: I will make sure to have two assistants when I have an emergency, not to exceed the points.
