Comments:
An announced initial inspection was conducted on Monday, May 6, 2019. There were no children in care during the inspection. The family day home is requesting a licensed capacity for five children age nine months through 12 years old. All required documents including the menu, emergency phone numbers, and emergency evacuation and shelter in place routes were posted. The family day home is a one level home with three bedrooms and two bathrooms. The living room and dining room will used for childcare. The first aid kit and emergency equipment were present and inspected. The program will offer snacks and meals to children in care. Medication will not be administered. Transportation is not provided. There are no pets in the household. The provider has submitted all of the required application materials at this time. During the inspection, compliance with some standards could not be determined. Compliance with these standards will be determined during unannounced visits once the program is in operation. Though some standards of care could not be determined during the initial study, the family day home is in substantial compliance with the standards that could be determined. A six-month conditional license will be recommended. During the conditional period, the center will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure. Description: Based on observation on 05/06/2019, the provider did not ensure that the first aid kit contained the required items. EVIDENCE: The first aid kit did not contain a thermometer and more than one triangle bandage.

Standard #: 22VAC40-111-770
Description: Based on observation on 05/06/2019, the provider did not ensure that a working portable battery-operated weather band radio shall be kept in a designated area and be available to caregivers at all times. EVIDENCE: There wasn't a battery operated weather band radio at the family day home.

Description: Based on observation on 05/06/2019, the provider did not ensure that the required posted numbers included the responsible person for emergency backup care. EVIDENCE: The phone number for the person identified for emergency back up care was not posted with the other posted emergency numbers.

Plan of Correction: Corrected on site. Posted the number during the inspection.

Description: Based on observation on 05/06/2019, the provider did not ensure that an operable thermometer available to monitor refrigerator and freezer compartment temperatures. EVIDENCE: The refrigerators and freezers used for the family day home did not have a thermometer.

Plan of Correction: Will order a thermometer on Friday and e-mail you to let you know when I have received it.

Description: Based on a review of caregiver and household member records on 05/06/2019, the provider did not ensure that a current Report of Tuberculosis Screening form was completed within the last 30 days. EVIDENCE: The most recent report of tuberculosis screening for Caregiver #1 and Household Member #1 is dated 07/11/2018.
