Technical Assistance:
Further reminded the provider that all occupied cribs must be spaced at least 12 inches apart. Discussed creating better management systems in order to come into compliance with all record keeping and background check requirements.

Comments:
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 4 children, totaling 13 points. Children's records, adult records, background checks, the physical plant, children's nutrition, emergency preparedness items, caregiver training, and administrative documents were inspected today. The children were observed playing with toys and interacting with each caregiver. The children were to be served macaroni and cheese, corn, strawberries, and milk for lunch today. This inspection was completed between approximately 10:00am and 11:30am. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on a review of children's ointments, it was determined that the provider has not obtained all required information for one child's diaper ointment. Evidence: One child's diaper ointment was not labeled with the child's name. In addition, the provider has not obtained written permission from the parent to apply the ointment when necessary.

Plan of Correction: I will ask the parent to label the item and to complete the form.

Standard #: 22VAC40-111-830-A
Description: Based on a review of emergency drill documentation, it was determined the provider has not conducted a monthly fire evacuation drill. Evidence: The provider did not conduct a fire evacuation drill during the month of April, 2019.

Plan of Correction: I will do 2 drills in May.

Standard #: 22VAC40-111-120-B
Description: Based on a review of records, it was determined that one caregiver record was not complete. Evidence: The file for assistant caregiver A does not include: age verification; a second reference check

Plan of Correction: The file will be updated and complete.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: Child record A does not include the last name for one emergency contact person. Child record B and record C do not include documentation showing that each parent has reviewed their child's contact information in the file.

Plan of Correction: The parents will be asked for the missing information.

Standard #: 22VAC40-111-70-A
Description: Based on a review of administrative documents, it was determined that the provider has not given all required written information to all parents. Evidence: Child record A does not include the parent's signature on the provider's Liability Insurance Declaration document.

Plan of Correction: I will ask the parent to sign the form.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of caregiver background checks, it was determined that the provider has not obtained one required background search within 30 days. Evidence: Assistant caregiver A began employment on 4/17/18. The provider has not submitted the Central Registry background search request for this employee.

Plan of Correction: I forgot to mail in the request. She completed the form and had it notarized last year, but I forgot to mail it. I will mail it immediately, within the required 10 days, and send a copy of the results to the Inspector.
