Standard #: 22VAC40-111-60-B Description: Based on record review, in two of six children's records reviewed, the provider did not ensure that each child's record contained all required information. Evidence: As confirmed by the provider, the record for Child #4 did not have documentation of the child's first date of attendance and the record for Child #5 did not have the work phone number for one parent. Plan of Correction: The record for Child #4 was updated during the inspection. Provider will contact the parent of Child #5 to get the phone number.
Standard #: 22VAC40-111-70-A Description: Based on record review, the provider did not ensure that before the child's first day of attendance, parents shall be provided with all required information in writing. Evidence: As confirmed by the provider, there was no documentation in the record for Child #5 whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia. Plan of Correction: The documentation will be signed today by the parent.
Standard #: 22VAC40-111-320-A Description: Based on observation and inspection of the family day home, the provider did not ensure that small electrical appliances such as, but not limited to, curling irons, toasters, blenders, can openers, and irons shall be unplugged unless being used by the caregiver or with children under close supervision. Evidence: As verified by the provider, a coffee maker and a bottle warmer were plugged in outlets in the kitchen and were not currently being used. Plan of Correction: The provider corrected during the inspection.
Standard #: 22VAC40-191-40-D-5-a Description: Based on record review, in one of six household member records reviewed, the provider did not ensure that a child protective services central registry check was completed on a household member within 30 days of a person in the home becoming 14 years old. Evidence: Provider confirmed that there were not results of a child protective services central registry check for household member #5 (date of birth 2/19/05). Plan of Correction: The document has been mailed and now just waiting on the results from the Office of Background Investigations. I will forward a copy to the licensing inspector.
