Technical Assistance:

The provider shall submit documentation of all remaining required training hours for herself and both assistant caregivers prior to the license expiration on 6/9/19.

Comments:

An unannounced full renewal inspection was conducted today. The provider and two assistant caregivers were caring for 9 children, totaling 22 points. Children's records, emergency preparedness items, caregiver/household records, background checks, health records, the physical plant, and the care of children were inspected today. The children were observed playing with toys, coloring pictures, playing outside, and interacting with each caregiver. The children were to be served chicken and rice soup with mixed vegetables, apples, and milk for lunch later today. This inspection was completed between approximately 10:15am and 12:15pm. Standard #:	22VAC40-111-800-B
Description:	Based on a review of emergency preparedness items, it was determined that the provider has not conducted an annual review of the emergency preparedness plan. Evidence: The provider most recently reviewed the emergency plan on 1/23/18.
Plan of Correction:	I will review the emergency plan with both of my assistants.
Standard #:	22VAC40-111-60-B
Description:	Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record A does not include the mother's work phone number. Child record D does not include the parent's medical insurance policy number.
Plan of Correction:	Both parents will be asked for the missing information.
Standard #:	22VAC40-111-70-A
Description:	Based on a review of administrative documents, it was determined that one parent has not been given all required written enrollment information. Evidence: Child record B does not include a signed "Medication Policy" statement.
Plan of Correction:	I will ask the parent to sign the form today.
Standard #:	22VAC40-111-80-A
Description:	Based on a review of children's records, it was determined that two records do not include verification of each child's proof of identification. Evidence: Child record B and record D do not include documentation showing that the provider has viewed each child's proof of identification.
Plan of Correction:	I will view each ID again and update the documentation.
Standard #:	22VAC40-111-170-A
Description:	Based on a review of health records, it was determined that one adult household member has not obtained a TB Health Screening. Evidence: Adult household member D has not obtained a TB test.
Plan of Correction:	I will make sure she gets her TB test done now that she is an adult.

Description:	Based on observations, it was determined that one child was seated in an infant seat without the safety strap fastened. Evidence: The Inspector observed one 5 month old child seated on an infant seat without the safety strap fastened.
Plan of Correction:	The belt was buckled immediately during the inspection.
Standard #:	22VAC40-191-40-D-5-a
Description:	Based on a review of background checks, it was determined that the provider has not completed one required background search for a family member living in the home. Evidence: In January, family member B became of the age that a Central Registry background search is now required. The provider has not completed her Central Registry background search.
Plan of Correction:	I submitted her CPS background search request on May 14, 2019. I called the CPS office during the inspection to see if they could send the results today, but the search has not yet been completed. Once I get the results, I will forward the documentation to my Inspector.
