Technical Assistance:
Both the provider and assistant will need to complete another hour of annual training by June 2019.

Comments:
An unannounced renewal study was conducted today. Seven children were present with two caregivers for seventeen points which is with ratios for supervision. Observed children playing outside upon arrival, playing with equipment and toys, diapering and infant care. The provider participates in the USDA food program. Lunch to be served today was a turkey sandwich, pears and milk. Space is used both on the main level, kitchen as an eating area and the basement play area. There is also a separate crib sleeping area for very young children. The household members and the caregivers have current background checks and TB documentation. Emergency preparation for drills, the first aid kit and first aid/CPR certifications are up to date. Areas of non-compliance are identified in the violation notice. Description: Based on review, the written emergency plan was not updated annually. Evidence: The plan was last updated and signed off 1/2018 and was not reviewed in January 2019.

Plan of Correction: Updated and signed during the inspection.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 1 of 2 children's records did not contain all required information for parents. Evidence: Child #1 was missing a signed acknowledgment of the Provision of Emergency Relocation and the home's medication policy.

Plan of Correction: Provider will have the parent sign the required forms.

Standard #: 22VAC40-111-90-C
Description: Based on record review, 1 of 2 children's immunization records was incomplete. Evidence: Child #2's record did not have an immunization update within 6 months as the last documented update was 8/8/2018.

Plan of Correction: Provider will request the parent update the immunization record.

Standard #: 22VAC40-111-240-A
Description: Based on observation, an area of the outside playground area is not maintained in a clean, safe or operable condition. Evidence: The wooden fence has a broken slat that is splintered in one spot.

Plan of Correction: Fence will be repaired.
