Standard #: 22VAC40-111-800-B Description: Based on review, the written emergency plan was not updated annually. Evidence: The plan was last updated and signed off 1/2018 and was not reviewed in January 2019. Plan of Correction: Updated and signed during the inspection.
Standard #: 22VAC40-111-70-A Description: Based on record review, 1 of 2 children's records did not contain all required information for parents. Evidence: Child #1 was missing a signed acknowledgment of the Provision of Emergency Relocation and the home's medication policy. Plan of Correction: Provider will have the parent sign the required forms.
Standard #: 22VAC40-111-90-C Description: Based on record review, 1 of 2 children's immunization records was incomplete. Evidence: Child #2's record did not have an immunization update within 6 months as the last documented update was 8/8/2018. Plan of Correction: Provider will request the parent update the immunization record.
Standard #: 22VAC40-111-240-A Description: Based on observation, an area of the outside playground area is not maintained in a clean, safe or operable condition. Evidence: The wooden fence has a broken slat that is splintered in one spot. Plan of Correction: Fence will be repaired.
