Comments:

An unannounced monitoring inspection was conducted between 8:30am -10:45am. Upon arrival there was 1 children in care, with 1 caregiver. A sample of 2 children and 3 caregiver records were reviewed. There were no children with food allergies at this time. Transportation procedures were observed and records reviewed.

Violations:

Standard #:	22VAC40-111-980-A
Description:	Based on observation, interview and a record review, the licensee did not have written permission that listed regularly scheduled trips for children that are transported by the licensee. Evidence: The licensee took child #1 in her vehicle and drove her to school, the morning of the inspection. Child #1's written permission slip did not indicate where the regularly scheduled trip was, or times of arrival or departure. The licenses stated she drives her to school ever day.
Plan of Correction:	The following corrective action was provided by the licensee during the exit interview: The licensee will have the child's parent's sign another field trip permission form that includes all required details of the trip.
Standard #:	22VAC40-191-60-B
Description:	Based on a record review, the licensee did not have all the required background checks: Caregiver #1 and #2 did not have the most recent sworn statement of affirmation completed.
Plan of Correction:	The following corrective action was provided by the licensee during the exit interview: The licensee will obtain the most recent statement and have both caregivers complete.
