- Technical Assistance:
Discussed the new trampoline in the backyard. The Provider understands it is not to be used during the FDH's hours of operations. The Provider stated it will never be used for children in child care.
An unannounced renewal inspection was conducted today between 9:55am and 12:10pm. 4 children (15pts) were present with 1 caregiver. Ratios were in compliance. The physical space, children's record, Provider/household member records, evacuation drills, medication compliance, attendance records and required posted documents were reviewed. Children were observed playing with age appropriate materials and interacting with the Provider. Interactions were positive. Areas of non-compliance are identified on the Violation Notice. For questions regarding this inspection, please contact email@example.com.
Standard #: 22VAC40-111-700-B Description: Based on record review and interview with the Provider, a long-term prescription was on site without written authorization from a doctor. Evidence: Child #1 required a long-term prescription form to be on file, signed by a physician. The form on file was for a short-term prescription (not to last more than 10 working days). Plan of Correction: I will ask the parents to get the new form right away.
Standard #: 22VAC40-111-320-A Description: Based on observation, small electrical appliances such as, but not limited to, curling irons, toasters, blenders, can openers, and irons were not unplugged unless being used by the caregiver or with children under close supervision. Evidence: The toaster and blender were plugged in on the upstairs kitchen counter. Plan of Correction: I will make sure they are unplugged each day. (The Provider unplugged the appliances while the Licensing Inspector was present.)
Standard #: 22VAC40-111-690-E Description: Based on observation, a soiled disposable diaper was not disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. Evidence: A soiled, wrapped diaper was left on the sink in the downstairs bathroom. Plan of Correction: The Provider stated when she went to throw it away she realized she didn't have a new liner in the foot-operated trash can. She typically keeps extras in the bathroom but was out. She was interrupted, before she was able to get a new liner from upstairs, and forgot to throw away the diaper. (The Provider obtained the extra liners from upstairs and threw away the diaper while the Licensing Inspector was present.)
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
