Comments:

An unannounced mandated Monitoring inspection was conducted today from 10:00am to 11:30am. There were 11 children ( 25 points) with 2 caregivers. Children were observed riding scooters w/helmets, coloring, singing songs, learning body parts in Spanish and French and having circle time. Reviewed 5 children's files and 3 caregiver files, attendance, emergency evacuation log, crib compliance documentation, menu, physical plant. Description:	Based on record review, written parent authorization was not obtained when topical skin products such as sunscreen, diaper ointment and lotion, oral teething medicine, and insect repellent are used. Evidence: Child #1 and Child #2 did not have written authorization for the use of topical skin products.
Plan of Correction:	I will get the parents authorizations.

Description:	Based on record review, children's record's were incomplete. Evidence: Child#1 was missing Annual documentation of the review of the child's emergency contact information. Last review January 2018.
