Technical Assistance:
The following were discussed with the provider: Monitoring the time that children spend in stationary equipment, methods for feeding infants and children's allergies.

Comments:
An unannounced monitoring inspection was conducted from 10:30am to 12:50pm. There were 9 children (23 points) in care of 3 caregivers. The children were observed playing with toys, getting diapers changed, eating lunch and napping. The lunch menu included chicken nuggets, bread, apples, sweet potatoes and milk. 6 children's records, 2 household member records and 3 caregiver records were reviewed. The emergency preparedness plan was also reviewed. There were no medications on site. Areas of non compliance are listed in the Violation notice. Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, children were allowed to drink while lying down. Evidence: Caregiver # 1 was observed feeding Child # 1 a bottle while Child # 1 was lying down in her crib.

Plan of Correction: Children will not be fed in this manner again.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the family day home cannot implement the documented allergy plan in children's records. Evidence: 1. Child # 2's record indicated that the he has a diagnosed food allergy that requires an antihistamine for mild symptoms and an emergency allergy medication for severe symptoms. 2. The provider cannot implement the allergy plan as none of the caregivers have the required training to administer medication and there are no medications on site for Child # 2.

Plan of Correction: We will get MAT trained and follow up with the inspector.

Standard #: 22VAC40-111-540-A
Description: Based on observation, safety straps were not used for an occupied high chair, while being used by a child. Evidence: Child # 3 was observed seated in a high chair. The safety straps were not fastened while Child # 3 was seated in the high chair.

Plan of Correction: The safety straps were fastened during the inspection.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water after diapering. Evidence: Caregiver # 1 did not wash Child # 1's hands with liquid soap and water after changing her diaper. Child # 1 was placed in a crib right after.

Plan of Correction: Caregivers will be trained about washing children's hands when required.
