Comments:
An unannounced monitoring inspection was completed on 5/15/2019. There were 11 children present from one year through three years old with the licensee and one assistants supervising. The point total was 22. Five enrolled children's records were reviewed, the provider's record was reviewed, and two assistant's records were reviewed. The inspector arrived at the provider's home at 9:40 am and departed at 12:30 pm. The children were observed during: free play time, dance, art, outside time, lunch and nap. Discussion with licensee about handwashing procedures.

Violations:
Standard #: 22VAC40-111-920
Description: Based on review of the menu, the licensee failed to ensure that a current menu was posted. Evidence: 1. The menu posted was dated 10/22/2018.

Plan of Correction: The licensee will update the menu.

Standard #: 22VAC40-111-100-A
Description: Based on review of five children's records, the licensee failed to ensure that all physical exams. Evidence: 1. Child # 5 started the program on 9/30/18 and the date of the physical exam was 11/8/2018.

Plan of Correction: In the future all physicals will be obtained before they start.

Standard #: 22VAC40-111-60-A
Description: Based on review of five children's records, the licensee failed to ensure that all records were complete. Evidence: 1. Child # 3 was missing the annual update according to the standard 22 VAC 40-111-780-B-1.

Plan of Correction: The licensee will have the parent to update all information today and will monitor all records.

Standard #: 22VAC40-111-70-A
Description: Based on review of five children's records, the licensee failed to ensure that before all child's first day of attendance that parent shall be provided in writing child's medical authorization form, information for parents, liability insurance, and provision for emergency. Evidence 1. Child # 2 did not have documentation that the parent received in writing the policy of administration of medication and provision of home emergency preparedness and response plan.

Plan of Correction: The licensee will have the parent to sign a copy and will be provided a copy today. In the future all parents will be given copy and a copy will remain in the file.

Standard #: 22VAC40-111-90-C
Description: Based on review of five children's records, the licensee failed to ensure that all immunizations were updated as required by the standard. Evidence: 1. Child # 3 was enrolled on 9/6/2017 with the DOB of 9/8/2016 and did not have immunization updated every 6 months until the child was 2 years old. The most recent immunization record on file was from 8/28/ 2017.

Plan of Correction: The licensee will have the parent to bring in a copy of the most recent immunization record and will monitor.

Standard #: 22VAC40-111-180-A
Description: Based on review of three staff's records, the licensee failed to ensure that all TB screenings and or test were completed every two years. Evidence: 1. Staff # 3 most recent TB test was completed on 3/6/2017.

Plan of Correction: The licensee stated that the staff person is not working today and has an appointment to obtain a TB test today. In the future all TB screening and or test will be completed within the two year timeframe.
