Technical Assistance:
Provided consultation: sand boxes should be covered when not in use; 6 foot perimeter from obstacles/structures is required around playground equipment.

Comments:
An unannounced renewal inspection was conducted from 9:30 - 11:00 am with the provider and her assistant. There were nine children in care, ranging in age from three years to eight years of age, for a total of 16 points. The children were observed having free play outside, eating snack, and engaged in Montessori lessons. Snack served today was cheese sticks, Nutrigrain bars and water.Childcare areas and supplies were observed. Four children's records were reviewed. Two provider/household records were reviewed and in compliance. The emergency drill log was reviewed. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Standard #: 22VAC40-111-830-A
Description: Based on a review of documentation, the provider did not have a record of the dates of the practice emergency evacuation drills for the past 12 months. Evidence: The record of practice drills could not be located during the inspection.

Plan of Correction: Will create a new emergency drill log and document monthly emergency escape and shelter in place drills.

Standard #: 22VAC40-111-830-B
Description: Based on a review of documentation, the provider did not have a record of the dates of the practice shelter in place drills for the past 12 months. Evidence: The record of shelter in place drills could not be located during the inspection.

Plan of Correction: Will create a new emergency drill log and document monthly emergency escape and shelter in place drills.

Standard #: 22VAC40-111-100-A
Description: Based on a review of four children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: child #3 (date of first attendance: 1/3/19) did not have documentation of a physical in their record.

Plan of Correction: Will ask the parent to provide the missing documentation.
