Technical Assistance:
The next anticipated inspection will be the provider?s renewal inspection. Provider was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDSS website.

Comments:
Today an unannounced Monitoring inspection took place between 10:15am and 12:10pm. There were 3 children present with the provider for a total of 6 points within the supervision guidelines. The children were observed playing freely with trucks and musical instruments, reading books, counting, watching videos on the provider's tablet, washing hands and eating lunch (chicken with rice, mixed vegetables and milk). A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical exam within 30 days after the first day of attendance for each child. Evidence: Child #2 (date of enrollment: 4/19/2019) and Child #3 (date of enrollment: 4/19/2019) files do not contain physical exams.

Plan of Correction: The provider contacted the head start program during the inspection to obtain the missing documents.

Standard #: 22VAC40-111-50-E
Description: Based on record review and interview, reports on children were not maintained for two years. Evidence: Attendance records available begin on April 24, 2019. The provider stated that she discards the attendance records.

Plan of Correction: I will create a folder to keep them in.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation of immunizations before each child may attend. Evidence: Child #2 file did not contain immunizations.

Plan of Correction: The provider contacted the head start program during the inspection to obtain the missing documents.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and interview, the findings of the most recent inspection were not posted. Evidence: The most recent inspection was not posted. The provider stated that she did not know she needed to post the inspection findings.
