Comments:
An unannounced monitoring inspection was conducted from 10:45 a.m. - 12:30 p.m. There were five children in care with the provider ages 2 years, 3 years, 3 years, 4 years, and 4 years for a total of 10 points. Lunch served today is chicken nuggets, french fries, mixed fruit, corn and milk. Five children's records were reviewed. The provider record was reviewed and in compliance. There are reportedly no medications being administered to children in the family day home at this time. The most recent emergency escape drill was conducted 05/14/2019. Standard #: 22VAC40-111-60-B
Description: Based on review of five children's records it was determined that not all records were complete. Evidence: The record for child A, start date 04/23/2019 did not have documentation of proof of identity.

Plan of Correction: The provider will request proof of identity from the parent of child A.

Standard #: 22VAC40-111-280
Description: Based on observation of the kitchen area it was determined that not all potentially poisonous substances were kept inaccessible from children. Evidence: A bottle of glass cleaner was observed on a metal shelf accessible to children. A hair product marked keep out of reach of children was observed on the kitchen table. Child B was observed beginning to open the container as the provider removed the product from the child's hands.

Plan of Correction: All potentially hazardous substances will be kept inaccessible to children.

Standard #: 22VAC40-111-680-A
Description: Based on observation of the family day home it was determined that not all children washed their hands before eating. Evidence: Five children were served lunch without washing their hands.

Plan of Correction: The provider ensure that all children wash their hands before meals.
