Technical Assistance:
Discussion about outdoor play equipment.

Comments:
An unannounced renewal inspection was conducted beginning at 10:30 a.m. and ending at 12:30 p.m. There were nine children in care ranging in age from thirteen months to five years of age for a total of twenty-one points. One child was picked up during the inspection for a total of eight children, twenty points. Younger children were observed having free play inside. A thirteen month old was observed napping. Older children were observed outside on the play equipment. After outdoor time, children returned inside and washed hands and had free play. After free play, children played Loterai (Spanish Bingo). After their group game, children prepared for lunch. Lunch today was macaroni and cheese, meatballs in tomato sauce, broccoli, strawberries and bananas and milk. Four children's records and two staff records were reviewed. Staff training was reviewed. The provider had documentation of 10 hours of training and has until 6/16/2019 to document 6 additional hours. The provider is not Medication Administration Training (MAT) and cannot administer medications to children in care. Description: Based on observation, monthly emergency drills were not documented. Evidence: There was no documentation of emergency drills for February 2019-April 2019.

Description: Based on observation, the weekly menu was not posted. Evidence: There was no documentation of a weekly menu

Standard #: 22VAC40-111-60-B
Description: Based on review of four children's records, not all records were complete. Evidence: Child #1, first day of attendance 10/1/2018, did not have two emergency contacts, individuals authorized for pick up and parent 2's employer contact information.

Plan of Correction: This information will be requested again from the parent.

Standard #: 22VAC40-111-240-A
Description: Based on observation, not all equipment of the family day home was maintained in safe, operable condition. Evidence: On the outdoor play structure the enclosed slide has separated and has a gap that creates an entanglement and scratching hazard. On the bottom of the structure, where the opening to the 'play house area' is there are eight staples exposed that create a puncture and scratch hazard and a screw at the bottom that extends approximately six thread lengths that create a puncture and scratch hazard. In the daycare area, there is a Bumbo seat that has torn foam all along the top of the seat.

Plan of Correction: The play equipment will be put off limits until it is fixed. The seat will be removed.
