Technical Assistance:

Again discussed the placement of cribs during nap. When cribs are occupied, they must be spaced at least 12" apart. The provider reports that when children use the cribs for nap, they are moved to allow at least 12" in between each occupied crib. The provider also reported that the children's floor seats are removed from the nap room when children are using the cribs in that room.

Comments:

An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 12 children, totaling 35 points. Children's records, new caregiver records, caregiver training, background checks, children's nutrition, children's programming, caregiver:child interactions, and the indoor/outdoor physical plant were inspected today. The children were observed playing outside, eating lunch, and preparing for their afternoon nap. The children were served macaroni and cheese, sliced strawberries, and milk for lunch today. This inspection was completed between approximately 11:45am and 1:00pm. Thank you for meeting with me today. Standard #:	22VAC40-111-60-A
Description:	Based on a review of administrative documents, it was determined that the provider does not have a complete enrollment file for each child in care. Evidence: The provider does not have a complete file for enrolled child C. In this child's file, the provider only has the child's immunization record and physical exam report.
Plan of Correction:	The child's older brother is also in care and I have his complete file. If I needed to contact the parents or an emergency contact person, I have that information available. I gave the enrollment forms to the parent. I will follow up to make sure the completed forms are returned tomorrow.
Standard #:	22VAC40-111-60-B
Description:	Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record A does not include the provider's signature to indicate that she viewed the child's original proof of identification. Child record B does not include the parent's medical insurance policy number.
Plan of Correction:	I saw the child's birth certificate. I will update the documentation. The parents will be asked for the missing information.

Description:	Based on observations, it was determined that two electrical outlets were not covered. Evidence: In the nap room, two outlets next to children's cribs were not covered.
Plan of Correction:	The outlets will be covered today.
