Comments:
A Subsidy Health and Safety inspection was conducted on May 30 2019 between 9:45am and 11:15am to review supplemental health and safety requirements. There were 5 children in the direct care of the provider. Children were observed working on puzzles, coloring, and preparing to go outside. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-170-B-12
Description: Based on observation, the provider does not have documented instructions from a physician regarding the food for which a child in care is allergic. Evidence: Child #3 record states the child has a food allergy. There is not instructions from a physician located in the family day home.

Plan of Correction: I will let the mother know so she can bring the doctor order.
