An unannounced renewal inspection was completed on May 28, 2019 from approximately 10:15a.m. to 12:15p.m. Three children, ages sixteen months through three years old, were in care for a total of seven points, with two caregivers. Caregivers and children were observed in the indoor space engaged in activities to include painting, diapering, toileting, hand washing, snack time and music and movement. Interviews and interactions were conducted and no medications are given. Five children?s records, three caregiver records and one household member record were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. Standard #: 22VAC40-111-780-B
Description: Based on a review of children?s records, the licensee failed to review with the parent annually the child?s emergency contact information to ensure the information is current for four of five children. Evidence: 1. Child #1, Child #2, Child #3 and Child #4's records, all enrolled between the years of 2013 and 2017, lacked annual reviews of the children's emergency contact information. 2. Caregiver #1 acknowledged these reviews have not been completed as required.

Plan of Correction: The provider will have all parents review their child's emergency contact information now and then begin to review all the children's information annually every February.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver records, the licensee failed to ensure that each caregiver obtain a tuberculosis screening every two years for one of three caregivers. Evidence: 1. Caregiver #2?s record contained a tuberculosis screening/test dated June 27, 2016 and there was no two year subsequent screening/test. 2. Caregiver #1 acknowledged this screening/test has not been obtained timely.

Plan of Correction: The provider will have Caregiver #2 obtain the TB.
