Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on May 28, 2019 from 12:45 p.m. through 2:00 p.m. There were nine children in the care of the provider for a total of sixteen points. Five children's records and the record for the provider was reviewed and updated. No medication is being administered at the present. Areas of non-compliance may be found on the violation notice. The home was toured, all emergency supplies were reviewed, and care of children was observed. Description: Based on record review, the provider failed to have the parent complete an annual review of emergency contact information. Evidence: 1. Five children's records were reviewed. 2. Child # 1, 2, 4 and 5 did not have documentation of obtaining an annual update of emergency contact information. 3. The provider confirmed she had not obtained annual updates for emergency contact information.

Plan of Correction: Emergency contact information will be requested and obtained form parents.
