Comments:

On May 28, 2019, a unannounced mandated inspection was completed between the hours of 10:30am and 1:00pm. There were two children in care and they were observed during water play activities, television time, lunch time, and nap time. Also during this inspection, provider and children's records were reviewed as well as the first aid kit and drill records.

Description:	Based on review of records, the provider failed to annually obtain the parent's acknowledgement of the record review. Evidence: Child's (2) record was missing parent's signature for acknowledgement of the record review.
Plan of Correction:	The provider will obtain documentation each year from parents showing acknowledgement of record review.
Standard #:	22VAC40-111-60-A
Description:	Based on review of records, the provider failed to ensure that records were complete. Evidence: Child (2) did not have have his/her concurrent school/program within his/her record.
Plan of Correction:	The provider will update records with necessary information.
