Technical Assistance:

An unannounced monitoring inspection was conducted on Tuesday, May 21, 2019 to determine the provider's compliance with licensing standards. The inspector arrived at 2:45 PM and concluded the inspection at 4:30 PM. The provider and her assistant were present with five children, totaling 14 points. Children and caregivers were observed engaging in activities to include napping, dancing, singing, reviewing the letters of the alphabet and the food pyramid. The provider had the required postings displayed. The first aid kit and emergency supplies were inspected. There are no medications being administered at this time. Five children's records, two caregiver records, fire drill, shelter in place and lockdown dills were reviewed. Violations were cited and are documented on the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-111-120-A
Description: Based on a review of caregiver records, the provider did not ensure that each caregiver's record contains all required information. Evidence: Caregiver #1 did not have documentation of the name, address and telephone number of a person to be notified in an emergency.

Plan of Correction: "I place to get emergency contact."

Standard #: 22VAC40-111-250-A
Description: Based on observation of the playground, the provider did not ensure that all hanging hazards shall be out of reach of children under five years of age. Evidence: 1. On the playground, a basketball hoop system with a base and sturdy steel pole had torn netting. 2. The provider cares for four children under five years of age.

Plan of Correction: "I did remove basketball net."

Standard #: 22VAC40-111-290
Description: Based on observation, the provider did not ensure that sharp kitchen utensils and other sharp objects shall be inaccessible to children unless being used by the caregiver or with children under close supervision. Evidence: 1. In the kitchen, one knife block with knives was on the kitchen counter accessible to the children in care. 2. The provider cares for five children; five years of age and younger.

Plan of Correction: "I did put knives up."
