Comments:
An unannounced renewal inspection was conducted on 05/30/2019 from 10:47am - 12:50pm. At the time of entrance there were 12 children in care with two caregivers. The ages of the children counted for a total of 30 points. Children were observed during morning program time which included lunch and preparation for nap. Transportation standards reviewed in addition to records for two caregivers and ten children. The entire home and backyard play area was reviewed. One violation was observed and is listed on the violation notice. This violation was discussed with the provider at the conclusion of the inspection.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that outside areas of the family day home shall be maintained in a safe condition. Evidence: 1. The inspector observed the following safety hazards in the backyard of the family home - which is used for outdoor play. a. Two cracks measuring between 3 and 6 inches were observed on the rock climber play structure. Cracked plastic is a pinch point for children's fingers. b. An AAA battery was observed on the ground near the shed. This is a choke hazard and could additionally harm a child if swallowed. c. Two metal garden boarder dividers were observed on the ground near the outdoor seating area. These dividers have sharp points. d. The extension cord for the outdoor patio lights was dangling down the side of the stair way used to enter and exit from the home. This is a trip hazard. e. A power charger strip was observed discarded on the ground beside the back door stairway. f. A plastic garbage bag was observed on the ground beside the children's equipment/playhouse area. Loose garbage bags are a suffocation hazard. 2. The backyard play area was utilized for outdoor play on the day of the inspection. When the inspector arrived the children had just come into the home from the backyard.

Plan of Correction: The cracks in the plastic will be repaired and/or covered to eliminate the pinch points. The battery, garden dividers, and garbage bag were discarded during the inspection. The power strip and extension cords were removed during the inspection.
