Comments:
An unannounced monitoring inspection was conducted today, June 3, 2019 as a follow-up to the issuance of the conditional license. The inspection was initiated at 11:00 am and concluded at 12:30 pm. Upon the inspectors arrival five children were in care with two caregivers for a total of twelve points. Children were observed listening to a story and reviewing colors and animal sounds with one caregiver. All areas used for child care were inspected. The family day home has age and stage appropriate materials. Today's lunch consisted of taco meat, soft shell, salsa, lettuce and milk. The following required information was posted: license, last inspection notice, and emergency numbers. The inspector interacted with the caregivers and children in care. Eight children's records were reviewed. Per the provider, currently medications are not being administered. The provider has MAT, CPR, and First aid training. The most recent evacuation drill was documented on 01/15/2019. Two shelter-in-place drills will need to be completed by the end of the year. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on review of the drill log and interview, the provider did not ensure the emergency evacuation procedures were practiced monthly with all caregivers and children during all shifts that children are in care. Evidence: 1. The most recent evacuation drill documented was dated 02/15/2019. 2.. There was no documentation of an evacuation drill for the month of March, April or May of 2019. 3. The provider stated she had not conducted a drill for March, April or May.

Plan of Correction: Per the provider: I will set a reminder on my phone for each month to make sure an evacuation drill is done.

Standard #: 22VAC40-111-100-A
Description: Based on review of children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. The record for child #5, enrolled 04/19/2019, did not have documentation of a physical examination on record. 2. The record for child #7, enrolled 02/12/2019, did not have documentation of a physical examination on record. 3. The record for child #8, enrolled 02/12/2019, did not have documentation of a physical examination on record.

Plan of Correction: Per the provider: I will ask the parents for the missing information right away.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's records, the provider did not obtain all required information for each child's record. Evidence: The record for child #5 (enrolled 04/19/2019) did not contain the following: addresses of two designated emergency contacts, policy number of the child's medical insurance, proof of the child's age and identity ; and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.

Plan of Correction: Per the provider: I will ask the parent to complete the missing information at pick up time.
