Comments:
An unannounced mandated monitoring inspection was conducted today from 10:15am to 12:00pm. There were 4 children present with 1 caregiver. Children were observed napping, having breakfast (cereal and milk), watching a video, practicing letters, coloring and reading books. Reviewed 4 children's files, 1 provider file, menus, attendance, 4 pet rabies, and physical plant. Violations of the Standards are recorded on the Violations notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on observation, children's files were incomplete. Evidence: Child#1 was missing Father's employment address and phone number, and Medial Insurance name and policy number. Child#2 was missing 1 emergency contact name, address and phone number.

Plan of Correction: I will get the information from the parents.

Standard #: 22VAC40-111-520-A
Description: Based on observation, cribs were not provided for children birth to 12 months. Evidence: Child#1 was observed sleeping in a pack n play playpen.

Plan of Correction: I will start using a crib for the child
