Inspection was attempted at 9:30 am. An unannounced mandated monitoring inspection was conducted today from 12:15pm to 2:00 pm. There were 3 children present with 1 caregiver. Reviewed 3 children's files, 1 caregiver file, 2 household member files, emergency evacuation log, attendance, menu, diapering procedure, hand washing procedure and physical plant. Children were observed outside on the front porch, playing ball, and watching a video. Violations of the Standards are recorded on the Violation notice. Standard #: 22VAC40-111-100-A Description: Based on record review, provider did not obtain a physical within 30 days of child's first day of attendance. Evidence: Child#3 (start date 2/27/2017) does not have a physical. Plan of Correction: I will get one from the parent.
Standard #: 22VAC40-111-60-A Description: Based on record review, children's files were incomplete. Evidence: Child#1 and Child#3 did not have documentation of annual review. (2/2018 respectively) Plan of Correction: I will have the parent's update.
Standard #: 22VAC40-111-180-A Description: BAsed on record review, provider did not obtain subsequent TB screening from adult household members every two years. Evidence: HH#1 had a TB screening that was expired (6/1/17, expired 6/1/19) Plan of Correction: I know she has one from the hospital. I will have her give it to me.
