Comments:
An unannounced inspection was conducted today, 6/6/19. There are no children are in care. A violation notice was completed regarding the violations found during the inspection. Should you have any questions please do not hesitate to contact your licensing inspector. Inspection began: 10:10 am and ended 11:40 am.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of staff records the provider did not have a current Tuberculosis Screening form on file. Evidence: 1. HHM #1 do not have a current TB screening.

Plan of Correction: HHM #1 is currently undergoing dialysis and will consult with his physician regarding this screening.

Standard #: 22VAC40-111-210-A
Description: Based on review of staff records the provider did not have the required hours of annual training. Evidence: 1. Staff #1 does not have the required 16 hours of annual training.

Plan of Correction: The provider will work on these hours next week and will ensure that she always maintains the 16 hours of annual training.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the home the provider did not ensure that all areas accessible to the children are maintained in a clean, safe, and operable condition. Evidence: 1. The grass in the yard that would be used by the children measures up to 24 inches high in some places. 2. The screen door to the day care room used by the children does not have a latch. 3. The floor in the day care room has rips/cuts and is not clean. 4. Located at the door to the day care room is a drill bit and screw.

Plan of Correction: 1. The providers son will cut the grass and ensure that it is maintained when children are in care. 2. The provider has a latch to put on the door. This will be replaced this weekend. 3. The provider has flooring that is supposed to replace the current flooring. This will be replaced within the next two weeks or prior to any children coming in care. 4. The provider will remove these items and ensure that all choking hazards and tools are put up away from children's access.

Standard #: 22VAC40-111-350-E
Description: Based on observation the water temperature exceeds the required range. Evidence: 1. The water temperature in the bathroom used by the children to wash hands measures 134 degrees F, exceeding 120 degrees F.

Plan of Correction: The temperature on the water heater will be turned down and will be monitored when children are in call.
