Technical Assistance:
As mentioned during the November inspection, please ensure that all information on the State model forms are completed and maintained in children's records. Discussion on the safety regarding the location of cribs and contents not permitted inside or around and within reach of children . Provider is in the middle of renovations of the kitchen area due to a flood. Please forward completed pictures of the repairs to licensing.

An unannounced monitoring inspection was conducted this date. A total of 9 children (21pts) with Provider and 1 Assistant present. One of the children is a household member and not counted in the licensed capacity. When the inspector arrived, the children were eating breakfast. Bagels and milk and apple sauce was served. Children transitioned to morning play on the middle level of the home. Children were observed cleaning up from morning play and lining up to go downstairs to the programming area in the basement. A total of 8 children's records and all staff and household records were reviewed. Record organization continue to need improvement. Previous violations from the November inspection were reviewed, one repeated systemic violation found along with other violations. Please forward the background check for the one staff to licensing for review once received. This inspection began at 10am and ended at 12:25pm.

Standard #: 22VAC40-111-960-B
Description: Based on observation of the programming area, an infant who can not hold their own bottle was not held while feeding. Evidence: Staff #2 did not hold Child #6 while bottle feeding. This infant is unable to hold their own bottle.

Plan of Correction: Staff picked up this child and held them while feeding.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, 5 out of 8 records were incomplete. Evidence: l. Child # 1's record does not contain a proof of identity, this is repeated and remains uncorrected from the November inspection. Child #1's record does not contain provisions of the emergency plan. Child #6's record does not contain documentation of proof of identity. 2. Child #3's record does not contain a documented start date of enrollment, insurance policy information, proof of identity, health exam, documentation of immunizations, Provider's medication policy and liability policy. 3. Child #4's and #6 record does not contain results of a physical exam. 4. Child #8's record does not contain documentation of results of health exam, immunizations, start date, proof of identity, medical insurance and policy number and physician's name and number.

Plan of Correction: I will be direct with parents regarding the information and documentation I need for the child records. Due dates will be provided to the parents.

Standard #: 22VAC40-111-250-E
Description: Based on observation of the crib area, a latex glove was found accessible to children. Evidence: Child #6 was sleeping in a car seat, located inside a crib. A latex glove was found inside the crib.

Plan of Correction: The glove was removed immediately and the baby was removed from the car seat.

Standard #: 22VAC40-111-430-C
Description: Based on observation of the kitchen area, a protective barrier was not found. Evidence: The steps located off the kitchen does not have a protective barrier to prevent children over the age of 2 yrs access to steps.

Plan of Correction: I will place a barrier at this area today.

Standard #: 22VAC40-111-530-F
Description: Based on observation of the crib area, a stuffed toy was found in an occupied crib. Evidence: A stuffed toy at the end of a pacifier was found in a crib with Child # 6.

Plan of Correction: We will remove this.

Standard #: 22VAC40-191-60-C-2
Description: Based on staff record review, one background check was not completed. Evidence: Staff #2 was hired on 4/23/18 and took a leave of absence. No Central Registry search was requested for this staff.

Plan of Correction: We will send the search request off by tomorrow. We will forward the results to the licensing inspector within 10 working days.
