The licensing inspector conducted an unannounced monitoring inspection on Friday, June 7, 2019 from 12:45pm to 2:15pm. The children were observed resting on mats during rest time. Today, there were 12 children, totaling 26 points, present with three caregivers. Five children?s records, four caregivers? records and one household record were reviewed during the inspection. Standard #: 22VAC40-111-170-A Description: Based on a review of caregiver records on 06/07/2019, the provider did not ensure that at the time of hire each caregiver has a current Report of Tuberculosis(tb) Screening form documenting the absence of tuberculosis in a communicable form. EVIDENCE: The record of Caregiver #4, whose date of hire was 05/27/2019, did not contain documentation of a tb screening. Plan of Correction: Staff got screening done today during the inspection.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records on 06/07/2019, the provider did not ensure that each caregiver obtained a current Report of Tuberculosis(tb) Screening documenting the absence of tuberculosis in a communicable form every two years. EVIDENCE: The tb screening in the record of Caregiver #1 is dated 12/02/2016. Plan of Correction: Will have staff go today to get updated tb screening.
