Comments:
An unannounced monitoring inspection was completed on 06/11/2019 from 9:15 A.M.- 12:15P.M. At the time of the inspection there were twelve children and two caregivers present. There was a total of twenty points. The records for six children, two caregivers, emergency plan, first aid kit and emergency supplies, required posted information, and refrigerator and freezer temperatures were reviewed. The children were observed during playtime, TV time, infant feeding, diapering, hand washing, arrival, free choice activities that included building and dramatic play, music and movement. Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of a physical examination within 30 days after the first day of attendance. Evidence: 1. Child #2's physical was dated 04/03/2019. First day of attendance was 03/01/2019. 2. Child #3's physical was dated 05/10/2019. First day of attendance was 04/01/2019. 3. The provider verified the dates on physical forms and the first date of attendance.

Plan of Correction: In the future the provider will track from the first day of attendance to one month of attendance as a reminder to obtain all physicals by due date.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children records, the provider failed to obtain immunization records before the first day of attendance. Evidence: 1. Child #2's immunizations were dated 04/03/2019. First day of attendance was 03/01/2019. 2. Child #3's immunizations were dated 05/10/2019. First day of attendance was 05/10/2019. 3. Provider verified dates the immunizations were dated and the first date of attendance.

Plan of Correction: In the future the provider will not let children begin until she has received immunizations for the child.
