Comments:
Thank you for your time and assistance with the announced initial inspection conducted today, June 10, 2019. The on-site inspection was held from 10:00a.m. to 1:30p.m. On this date, there were no children in the care of the provider and no children enrolled in the program. The records of the provider and one assistant/household member were reviewed. The provider will not be administering any medications at this time. The home and outside play areas were toured. There was an ample supply of toys and equipment for the children. The exception to compliance was recorded on the violation notice and the risk rating for the violation was noted on the supplemental page. Some items reviewed and observed included: required posted information, refrigerator and freezer temperature, emergency supplies, first-aid kit, warm water temperature, and daily attendance record. A recommendation will be made to issue a conditional family day home license with a capacity of 12 children and an age range of 2 years through 12 years. Violations:
Standard #: 22VAC40-111-770
Description: Based on observation and verification from the provider, the provider failed to have a battery-operated radio present. Evidence: 1. There was not a portable battery-operated weather band radio in the home. 2. The provider confirmed that he did not have a working radio present.

Plan of Correction: We will purchase the battery-operated weather band radio as soon as possible. We will take a picture of it and send it to the licensing inspector.
