Technical Assistance:
To assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery such as whole hot dogs sliced into rounds, nuts, seeds, raisins, uncut grapes, uncut raw carrots, peanuts, chunks of peanut butter, hard candy, and popcorn shall not be served to children under four years of age, unless the food is prepared before being served in a manner that will reduce the risk of choking, i.e., hot dogs cut lengthwise, grapes cut in small pieces, and carrots cooked or cut lengthwise. Hot dogs and grapes: if served to children aged 16 months to 4 years, must be cut into pieces no larger than ? inch in diameter.

Comments:
An unannounced monitoring inspection was conducted beginning at 12:00 p.m. and ending at 1:30 p.m. with the provider. There were five children in care ranging in age from ten months to four years of age for a total of thirteen points. Children were having music time at the beginning of the inspection. An infant was observed in an infant play area with a variety of toys. After music, children had free play with a variety of toys including a play kitchen and puzzles. After play, children washed hands and prepared for lunch. Lunch today was hot dogs, corn, apples and milk. Four children's records, one household member record and two caregiver records were reviewed. Emergency drills were reviewed. The provider is Medication Administration Training (MAT) certified however, no children in care have medications on site. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on review of documentation and interview with the provider, practiced emergency drills were not documented every month. Evidence: There was no documentation of emergency drills for March 2019, April 2019, May 2019.

Plan of Correction: The drills were practiced but the documentation has been misplaced.

Standard #: 22VAC40-111-900-A
Description: Based on observation, food that is considered a potential choking hazard was served to children in care. Evidence: Children age 23 months through 4 years were served whole hot dogs for lunch.

Plan of Correction: The hot dogs were cut during the inspection. In the future, they will be sliced before being served to children.
