Comments:
An unannounced monitoring inspection was completed on May 29, 2019 from approximately 2:00p.m. to 3:45p.m. Two children, preschool age, were in care, with one caregiver for a total of four points. The caregiver and children were observed in the indoor space engaged in activities to include, arising from nap time, snack time and free play. Interviews and interactions were conducted and no medications are given. Five children?s records and two caregiver records were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. Standard #: 22VAC40-111-780-B
Description: Based on a review of children?s records, the licensee failed to review with the parent annually the child?s emergency contact information to ensure the information is current for one of five children. Evidence: 1. Child #5's record, enrolled on May 3, 2016, lacked any annual reviews of the emergency contact information.

Plan of Correction: The provider will have the parent update the record.

Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency preparedness plan, the licensee failed to review the plan at least annually and update the plan as needed. Evidence: 1. The emergency preparedness plan was reviewed and lacked any documented reviews. 2. Caregiver #1 acknowledged annual reviews have not been documented.

Plan of Correction: The provider will begin to review the plan annually and document the reviews.

Standard #: 22VAC40-111-940-A
Description: Based on observation, the licensee failed to ensure that temperatures shall be maintained at or below 40 degrees Fahrenheit in refrigerator compartments. Evidence: 1. The refrigerator temperature was observed at 52 degrees Fahrenheit. 2. Caregiver #1 acknowledged the temperature was too high.

Plan of Correction: The provider will adjust the temperature in the refrigerator.
