Comments:
An unannounced monitoring inspection was completed on June 10, 2019 from approximately 1:30p.m. to 2:45p.m. Ten children, five months to nine years old, were in care with two caregivers, for a total of twenty-five points. Caregivers and children were observed in the indoor space engaged in activities to include free play, nap time, toileting and hand washing. Interviews and interactions were conducted and no medications are given. Five children?s records, two caregiver records and one household member record were reviewed. The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. Standard #: 22VAC40-111-780-B
Description: Based on a review of children?s records, the licensee failed to review with the parent annually the child?s emergency contact information to ensure the information is current for one of five children.
Evidence: 1. Child #1's record, enrolled on March 26, 2018, lacked documentation of an annual emergency contact review. 2. Caregiver #1 acknowledged this review has not been completed.

Plan of Correction: The provider will have the parent review the information.

Standard #: 22VAC40-111-280
Description: Based on observation, the licensee failed to ensure that potentially poisonous substances were stored in areas inaccessible to children.
Evidence: 1. Two bottles of spray paint were observed, unlocked and accessible to children, on an open shelf in the kitchen. 2. Caregiver #1 acknowledged the spray paint was not stored in an area inaccessible to children.

Plan of Correction: This was correced on site. The provider moved the paint to a location inaccessible to children.
