Comments:
An unannounced monitoring inspection was conducted today, June 14, 2019 to determine the providers compliance with family day home standards. The inspector arrived at 9:30 am and concluded the inspection at 11:30 am. Upon the inspector's arrival there were no family day home children present. The provider's own three children were in care. The provider had activities set up that included sand, coloring and tablets. Family day home children were expected to arrive later in the day. All areas used for child care were inspected including the rear yard. Children do not use the rear deck. The family day home has age appropriate toys and materials in operable condition for children in care. The first aid kit along with non-medical emergency supplies, and the emergency preparedness plan were all reviewed and checked for compliance. The family day home had the daily schedule, menu, license, most recent inspection notice, attendance sheets and emergency numbers posted. A smoke detector and fire extinguisher were observed. The inspector interacted with the provider and her children. One caregiver record and four children's records were reviewed. Medication is not being administered. The provider has current CPR and first aid training. The last documented emergency evacuation drill was held on 06/04/2019. The two required shelter-in-place drills were completed on 04/08/2019 and 05/19/2019. A lock down drill was documented on 06/04/2019. Per the provider, transportation is not provided. Today's lunch menu consisted of eggs, green beans, peaches, bread and milk. If you have questions regarding this inspection, please contact the licensing inspector at 804-662-9758.

Description: Based on review of four children's records, the provider did not obtain all required information for one child's record. Evidence: The record for child #2, enrolled 09/04/2018, did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.

Plan of Correction: Per the provider, I will have the parent complete the missing information as soon as they arrive.
