An unannounced renewal inspection was conducted on 6/17/19 from 11:45am until 1:15pm. At the time of entrance, there were 6 daycare children, plus 2 household children, in care, for a total of 13 points, with the provider, one assistant and one volunteer. A sample of 5 children's records and 4 provider/assistant/household member records were reviewed. Children were observed playing outside, eating lunch and resting quietly during nap time. Hand washing and lunch preparation procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and and required family day home postings were reviewed. The provider stated there are currently no children in care that require medication and there have been no accidents or injuries since the last inspection. Two violations in the areas of administration and physical environment and equipment are identified on the violation notice and were discussed with the provider during the exit interview.

Description: Based on record review and interview, the provider did not ensure that children's records contain all the required information. Evidence: 1. The record for child 1 contains one name, address and telephone number of designated persons to contact in case of an emergency if the parent cannot be reached, where two are required. 2. The provider confirmed that the record for child 1 contains only one emergency contact.

Plan of Correction: The provider will have the parent provide the second emergency contact for child 1 this afternoon.

Standard #: 22VAC40-111-240-A
Description: Based on observation and interview, the provider did not ensure that areas of the family day home shall be maintained in a safe condition. 1. There are 5 protruding nails sticking out of the back portion of the wooden fence in the backyard. The nails are approximately 2 inches in length and are accessible to the children in care. 2. The provider confirmed that there are protruding nails sticking out of the fence in the backyard.

Plan of Correction: The provider will ensure the nails are hammered down this evening and will have the fence repaired as soon as possible.
