Violations:
Standard #: 22VAC40-111-850-A
Description: Based on interview, the family day home failed to inform the department within 24 hours of the circumstances surrounding a serious injury to a child. Evidence: 1. The inspector received a telephone call from the provider on June 7, 2019. The provider reported that on May 30, 2019, child #1 received a dislocated elbow while in care at the family day home.

Plan of Correction: The provider completed an injury report at home when the accident happened. The provider thought that she only needed to complete an on-line report within 48 hours and was unable to make the report because her internet services were down. In the future, the provider will inform the department within 24 hours if a child in care receives a serious injury.
