Comments:
An unannounced monitoring inspection was conducted on Monday, June 17, 2019 from approximately 10:15 AM until 12:00 PM. The focus of the inspection was to determine whether the provider had corrected or is in the process of correcting previously cited violations in the areas of standards referenced above. During the inspection, children were observed preparing for lunch with hand washing. Today's lunch consisted of chicken nuggets, rice, green beans and milk. After lunch children had nap time. Caregivers sat with children and rubbed their backs while playing soft music. The family day home was was equipped with toys and supplies and items were available to the children. All areas used for child care, inside and outside, were inspected. The family day home has age and stage appropriate materials. The first aid kit and non-medical supplies were inspected and found complete. The family day home had the required information posted. The inspectors interacted with the caregivers and children in care. Required postings were reviewed and found complete. Three children's records, two caregiver records, and one household member's record were reviewed today as a follow-up. Caregivers have current First aid and CPR. Medications are not administered. The violations cited during the licensing period were checked for corrections. There was one repeat violations cited during today's inspection. Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not ensure all areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Evidence: 1. A window unit air conditioner in the main room used for child care was not attached to the window frame in a secure manner. Packing tape was used around the air conditioner unit to attach it to the window frame. 2. Two napping mats were torn in a way that exposed the interior foam.

Plan of Correction: Per the provider: We will mount the air conditioner to the window with screws to secure it. I will order new matts for the two children today.

Standard #: 22VAC40-111-560-A
Description: Based on observation, the provider did not ensure direct care and supervision of each child to include monitoring of each infant by placing each infant for sleep in a location where the infant is within sight and hearing of a caregiver; by in-person observation of each sleeping infant at least once every 15 minutes; or by using a baby monitor. Evidence: 1. At approximately 11:27 am, child #1 (9 months in age), was observed sleeping in a crib located in the living room outside of sight and hearing of the two caregivers. 2. The two caregivers were observed in the main child care room at the rear of the house. 3. The provider stated she checks on infants sleeping in the living room every 15 minutes. 4. At approximately 11:45 am, caregiver #1 and caregiver #2 had not checked on the sleeping infant, exceeding 15 minutes.

Plan of Correction: Per the provider: I will set reminders on my phone to check on sleeping infants and i will look for a monitor that can be used.
