An unannounced non-mandated inspection was completed on June 25, 2019. This focus inspection was in regards to administration , medication administration, emergencies, nutrition, care of children, physical environment and equipment , caregiver training and personnel. The children were observed eating lunch, dancing and washing hands. There were ten enrolled children present, two household members and two staff present (31 points). Areas of non-compliance are identified on the violation notice.

Standard #: 22VAC40-111-100-A
Description: Based on review, provider did not ensure 1 of 5 children's records had a copy of a physical examination within 30 days of child beginning attendance. Evidence: During the inspection conducted on June 25, 2019, the record for child #3 ( start date 05/9/2019) did not have documentation of a physical examination.

Plan of Correction: The provider will remind the parent again we need the physical or child will not be able attend until it is received.
