Comments:
An unannounced renewal visit was conducted today between the hours of 10:30am through 12:00pm with the provider on site. There were 7 children (16 points) with the provider. A sample of 5 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, games and tv time. Interactions between the children and provider were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, one of five children's records were not complete. Evidence: 1. Child #3 began with the provider on 6/18/2018 and did not have written authorization for emergency medical care should an emergency occur.

Plan of Correction: I will ask the parent to sign it today.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider did not provide the required information prior to the child's first day of attenance. Evidence: 1. Child #4 (start date 11/16/2018) did not have a signed copy of the information for parent form in the child's file.

Plan of Correction: I will have the parent review and sign today.

Standard #: 22VAC40-111-210-A
Description: Based on record review, the caregivers did not obtain a minimum of 16 hours of training annually. Evidence: 1. The provider had completed 2.5 hours of training in 2018. The provider did have 8.5 hours of training in 2019. 2. The Assistant completed 6 hours of training in 2018.

Plan of Correction: I will make sure we complete 16 hours of training this year.
