The licensing inspector conducted an unannounced monitoring inspection form 9:30am - 11:30am. There were five children in care (two residential) for a total of six points with one caregiver. The children were observed watching cartoons and playing outside. Three children's records, one caregiver's record and three household member's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-60-A Description: Based on record reviews, the provider failed to have an up-to-date record for one enrolled child. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child #3) was incomplete without up-to-date information. Plan of Correction: The provider will ask the child's parent to complete the necessary documents at pick up time today.
Standard #: 22VAC40-111-70-A Description: Based on record reviews, the provider failed to give the parents all required information in writing. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child #3) did not contain any documents that the providers is suppose to provide the parents and ask that they read and sign. Plan of Correction: The provider will give these documents to the parents today and ask that they read and sign.
Standard #: 22VAC40-111-90-C Description: Based on record reviews, the provider failed to obtain documentation of updates to child's immunizations. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child record #1) did not contain updated information for immunizations every six months for child under two. Plan of Correction: The provider will ask the child's parents again for the child's updated immunization information today.
Standard #: 22VAC40-111-260-C Description: Based on observations, the provider failed to empty a wading pool after use. Evidence: 1. The inspector observed the outside play area. There was a wading pool half full with water in the yard. The children did not have access to the pool and were playing on the gated deck. Plan of Correction: The provider failed to empty the wading pool after the last use. In the future, the provider will empty the wading pool after each use.
Standard #: 22VAC40-111-460-C Description: Based on record reviews, the provider failed to have a pet with current vaccinations. Evidence: 1. The inspector reviewed two pet's records. Pet #1's rabies vaccination expired 11/30/2018. Plan of Correction: The provider will have the pet's vaccinations updated and monitored to ensure they remain current.
